New Patient Forms
Many people are surprised to find out that
many common podiatry treatments are cov-
ered by their insurance plan. Although podi-
atrists are considered specialists, in many
cases a referral is not necessary for
treatment.
Below is a list of the major insurance
providers whose plans we accept. If you are
unsure whether your plan is accepted or not,
please call our office or your insurance
provider to confirm.
If you have an HMO, please give us a call as
most of them require a referral from your
primary physician.

office policies. Please review them prior to your
visit.
If you are preparing to visit us for the first
time, you can save a few minutes at your
appointment by printing and completing our
new patient registration form.
It is also important that you bring the
following with you to your appointment:

List of Current Medications (if any)
On-Time Policy: In order to maintain our
schedule and keep appointments on time for all
patients, we also require that anyone 10
minutes or more late for their appointment
reschedule to another available time.
